We hereby inform you that the undersigned VIPMED INTERNATIONAL S.R.L. (“Vipmed”) with headquarters in Drumul Gura Siriului str., no. 65A, et. 3, ap. 18, Sector 3, Bucharest, having the unique registration code no. 21752464 and order number in the trade register J40/9526/2007 processes your personal data, respectively:
- surname and first name;
- date of birth;
- the image (where applicable);
- series and number of the identity document;
- signature;
- state of health;
(hereinafter “Personal Data“).
Personal Data will be processed by Vipmed for the purpose of providing the services requested by you, respectively: the supply,of to the doctors of the clinics abroad with which Vipmed collaborates, of the specialized medical consultations, regarding the health problems you are facing.
We mention that, since the specialized medical consultations are provided by various doctors in clinics abroad, the Personal Data will be transferred abroad, respectively to: Austria, Germany, France, Switzerland, Turkey and the United States of America (as applicable) .
We also mention that Vipmed will be able to disclose the Personal Data to the entities with which it collaborates in order to facilitate the provision of the specialized services requested by you (for example, for the translation of the documents you provide to be later transmitted to clinics abroad).
Hereby, we inform you of the rights you have as a data subject in the context of the processing of Personal Data according to the provisions of Law no. 677/2001 for the protection of individuals regarding the processing of personal data and the free movement of such data (“Law 677/2011“), as follows: (i) the right to information (art. 12 of Law 677/2001); (ii) the right of access to data (art. 13 of Law 677/2001); (iii) the right to intervene on the data (art. 14 of Law 677/2001); (iv) the right of opposition (art. 15 of Law 677/2001); (v) the right not to be subject to an individual decision (art. 17 of Law 677/2001); and (vi) the right to go to court (art. 18 of law 677/2001).
To exercise these rights, you can send a written, dated and signed request to VIPMED INTERNATIONAL S.R.L., Drumul Gura Siriului str., no. 65A, et. 3, ap. 18, Sector 3, Bucharest.
We inform you that the processing of Personal Data takes place for the entire duration of the contractual relationship between you and VIPMED, upon completion of this relationship, the data thus processed will be destroyed.
Your refusal to give your consent for the processing of Personal Data in part or in whole determines the impossibility of Vipmed offering the services requested by you.
